Position Purpose:
The personal care assistant/worker is responsible for providing various care services to program participants including in center care and home needs to assist with activities of daily living
This role serves the underserved geriatric population by meeting their needs in the community to help with maintaining their independence in the home setting.
The position is Monday-Friday with rotating weekend and holiday coverage.
Position Details:
This position will primarily consist of home care assignments. However, occasional facility work will also be required.
Position will require working one weekend a month and rotating holiday coverage.
What you will do:
* Obtain vital signs and blood glucose readings as ordered
Provide personal care assistance with activities of daily living including toileting, feeding assistance, showers and grooming in the home and facility
Provide in center laundry services including washing and folding clothes
Assist in identifying signs and symptoms indicating physical, emotional or behavioral changes in participants and report abnormal findings to the clinic
Escort participants to appointments as necessary
In home personal care services
In home light housekeeping
In home light meal prep
Assignments rotate between in facility and home services.
Minimum Qualifications:
* High School Diploma or equivalent required.
* Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
* Experience in a long-term care facility or with a community-based geriatric program preferred.
* Must be action-oriented, manage conflict well, be customer focused, have high decision quality and flexibility to adapt to ongoing change.
* Ability to work with minimal supervision and exercise independent judgment.
* Superior written and verbal communication and presentation skills appropriate for audience comprehension.
* Valid drivers license and use of personal vehicle for home care visits
* Excellent organizational skills and the ability to perform multiple duties and functions related to daily operations while maintaining excellent customer service.
* Position may require 10% travel within the Centers geographic region and use of personal vehicle.
